Countries wishing to apply to GAVI for support, or to their report immunisation results, should use the correct GAVI forms.

Up to now these documents have been downloadable from this page. However, GAVI is phasing out these downloads and eventually all countries will be able to apply for GAVI support directly online.

Currently countries applying to GAVI in English or French for support for immunisation services (ISS), injection safety (INS) and new and underused vaccines (NVS), should now apply on-line at this address.
