Guidelines and forms

Countries wishing to apply to GAVI for support, or to their report immunisation results, should use the correct GAVI forms.

Up to now these documents have been downloadable from this page. However, GAVI is phasing out these downloads and eventually all countries will be able to apply for GAVI support directly online.

Currently, countries applying to GAVI in English or French for immunisation services support (ISS), injection safety (INS) and/or new and underused vaccines (NVS), should apply online using their username and password. If you do not have access, please write to proposals@gavialliance.org.

For other types of support, countries should use the forms listed below.
