The governments intends to introduce the vaccine against hepatits B into the routine EPI from 2006 as component of the tetravalent vaccine, including the antigens against DTP. The strategies to increase immunisation coverage are included in the multi-year strategic plan. The approach adopted by the Government is the “Reach Every District” approach, which consists in strengthening supervision combined with training, improving monitoring methods, establishing closer links between the health services and the community, and upgrading the planning and management of resources at district level.

The government seeks GAVI support to strengthen routine immunisation (ISS) and introduce a new combination liquid tetravalent vaccine (DTP-Hib). Despite an excellent health infrastructure and national information health systems, the immunisation service delivery systems are struggling to maintain adequate coverage levels for different antigens included in the schedule due to several issues identified in previous EPI reviews: poor systematic outreach, poor community demand, poor transport and communication systems.

The government is applying for GAVI support to introduce Hib vaccine into its EPI, and for INS. The proposal outlines targets and planned expenditures. GAVI support for Hib vaccination will open a new avenue for further improvement of preventable disease control, reporting and surveillance system and cost effectiveness of the national immunisation programme.

In view of intermediate to high endemicity of hepatitis B in India, the benefits provided by hepB immunisation would be substantial. One analysis, assuming a cost of $0.50 per dose, found that routine use of hepatitis-B vaccine would save 183,000 lives in each birth cohort, and would cost $947 per death averted (discounted) and $64 per years of life saved (discounted).

The audit by two external firms and two national consultants found that the verification factor for the reporting on immunisation coverage rates is nearly 100%, setting it at 0.999. The researchers come to a very positive judgement on the performance of Yemen's EPI and health staff. Completeness of reporting was set at 95.5% and vaccine wastage at 0%. No vaccine stockouts have been registered.

The researchers see an improvement compared to the previous audit and see some progress in the EPI. One of the major issues is the lack of monitoring of vaccine wastage.
